Long-Term Follow-Up of Chronic Hepatitis C Patients Treated with Interferon-Alpha: Risk of Cirrhosis and Hepatocellular Carcinoma in a Single Center over 10 Years

چکیده
Objectives: Interferon (IFN)-based therapy for chronic hepatitis C (CHC) is cost-effective and is associated with reduced risk of disease progression. We aimed to assess the incidence of cirrhosis and hepatocellular carcinoma (HCC) and to identify risk factors associated with disease progression. Methods: We retrospectively reviewed 280 CHC patients who were registered at our hospital between 2001 and 2010. Results: About 80% of patients received antiviral treatment. The 10-year cumulative incidence of cirrhosis was significantly lower among patients who received antiviral therapy than among those who did not (8.3 vs. 44.0%; p = 0.001). Among them, patients with sustained virological response (SVR) had a significantly lower incidence of cirrhosis than those without SVR (0.6 vs. 33.9%; p < 0.001). Cox proportional hazards regression showed that SVR was the significant independent factor for reducing the risk of cirrhosis (hazard ratio, HR = 0.03; p = 0.034). The 10-year cumulative incidence of HCC was higher among patients who did not receive antiviral therapy than among those who did (43.9 vs. 6.1%; p < Received: November 18, 2013 Accepted after revision: October 18, 2014 Published online: January 10, 2015 Jong Eun Yeon, MD, PhD Department of Internal Medicine, Guro Hospital Korea University College of Medicine 97, Guro-Dong Gil, Guro-Dong, Guro-Ku, Seoul 152-703 (South Korea) E-Mail jeyyeon @ hotmail.com © 2015 S. متن کاملComparison of hepatitis C virus risk factors in genotypes 1a and 3a
Background: One of the most important causes of chronic liver disease is hepatitis C virus (HCV), which causes liver cirrhosis and hepatocellular carcinoma. To control the prevalence of the disease, knowledge and information in risk factor of HCV are required. The aim of this study was to compare the risk factors of infection between HCV patients with genotypes 1a and 3a. Methods: This is an o...
متن کاملEvaluation of serum AFP (α-fetoprotein) level in HBsAg carrier patients for diagnosis of hepatocellular carcinoma
Background: Hepatocellular carcinoma (HCC) is common all over the world as well as Iran. The incidence of HCC is higher in hepatitis B carriers and it is highly recommended to periodically screen these patients by serum alpha-fetoprotein (AFP) and liver ultrasound (US) every 6 months. We explore the diagnostic accuracy and the performance of (AFP) in cases of hepatitis B carriers as a scree...
متن کاملLate HDV RNA relapse after peginterferon alpha-based therapy of chronic hepatitis delta.
UNLABELLED Interferon alpha is the only treatment option for hepatitis delta virus (HDV). Trials investigating the efficacy of pegylated interferon alpha (PEG-IFNa) showed HDV RNA negativity rates of 25-30% 24 weeks after therapy. However, the clinical and virological long-term outcome of HDV-infected patients treated with PEG-IFNa is unknown. We performed a retrospective-prospective follow-up ...
متن کاملSAFETY AND EFFICACY OF INTERFERON ALFA FOR THE TREATMENT OF CHRONIC HEPATITIS C INFECTED SUBJECTS WITH TRANSFUSION DEPENDENT THALASSEMIA IN IRAN
Up to 30% of Iranian adult multi-transfused thalassemic patients are infected with hepatitis C virus (HCV) which can intensify the progression of liver disease caused by iron overload in this group of patients. Our aim was to assess the biochemical and virological response of interferon alfa (INF-α) and its safety in thalassemic patients with chronic HCV infection. This trial was a single c...
